Job Summary: 
The Behavioral Health Navigator plays a vital role in supporting patients enrolled in the Behavioral Health Integration (BHI) program. This team member is responsible for identifying, monitoring, and engaging patients with behavioral health needs, such as depression, anxiety, or chronic stress through proactive outreach, care coordination, and education.
Using structured screening tools and protocols, the BHI Navigator ensures patients receive timely support and are connected with the right services within the practice and externally as needed. This position is ideal for someone passionate about mental health, patient advocacy, and collaborative care.

Responsibilities

Patient Monitoring and Outreach:
  • Establish and maintain a strong rapport with patients, promoting trust and open communication to help coach to improve mental health through goals and education.
  • Review alerts daily from screening tools such as PHQ-9 and GAD-7.
  • Contact patients who meet BHI criteria to discuss symptoms, assess needs, and offer support or resources.
  • Provide regular check-ins with enrolled patients to monitor progress and adherence to care plans.
Health Education and Care Coordination:
  • Educate patients about their medical conditions, treatment options, and self-management techniques.
  • Collaborate with clinical providers, behavioral health consultants, and chronic care teams to support whole person care.
  • Schedule follow-up appointments, communicate patient concerns to providers, and ensure coordination of care.
  • Perform “wellness calls” for patients based on PHQ and GAD7 scores.
  • Recognize and respond appropriately to urgent/emergent situations per SOP.
  • Assess patients for suicidal ideation and activate proper emergency response when needed.
  • Establish and maintain effective working relationships with physicians and clinic staff.
  • Responsible for placement and management of behavioral health referrals.
  • Help patients understand the BHI program and their role in their own care.
Compliance and Documentation
  • Maintain accurate and up-to-date patient records confidentially.
  • Ensure that documentation supports CMS billing requirements for BHI services.
  • Conduct monthly audits for billing behavioral healthcare services.
  • Track time spent on eligible patient care activities and contribute to accurate monthly billing processes.
Compliance and Training:
  • Stay current on healthcare regulations, industry trends, and best practices.
  • Attend training sessions and participate in continuing learning and development.
  • All other duties assigned; including duties performed for affiliates, assigns, lessees, contractors or other third parties.

Skills & Qualifications

  • Minimum of 1-2 years of experience in healthcare, behavioral health Previous mental health experience preferred.
  • Experience in and a passion for healthcare and improving patient outcomes.
  • Strong understanding of medical terminology, healthcare processes, and mental health.
  • Personal, outgoing, and compassionate.
  • Ability to work independently and demonstrates the ability to collaborate and communicate effectively in a healthcare team setting.
  • Highly proficient with computers especially: EMR Software, Microsoft Suite
  • Excellent written and spoken communication skills required.
  • Ability to maintain effective and professional relationships with patients and other members of the care team.
  • Some flexibility in work hours may be necessary to accommodate patient needs, including evenings or weekends.
Education:
  • Licensed Practical Nurse (LPN) with behavioral health background, Associate’s or Bachelor’s Degree in Social Work, Psychology, Health Sciences, or a related field preferred.

Physical Requirements:
The physical demands of this position will include sitting and standing with occasional light to medium lifting. An employee must meet these requirements to successfully perform the essential duties of this job. Reasonable accommodation will be made in accordance with ADA rules and regulations.
